- Full Name
- Email address
- Mailing Address
- The device that may need replacement ex. Misfit activity tracker, iHealth blood pressure cuff, iHealth glucometer, iHealth pulse oximeter
- Details of the issue
- Was the device lost or stolen?
- If there is something wrong with the device, how is the device malfunctioning? How did it break?
